TNO is actively searching for Homemaker/Cleaner to join a new program Personal Support Services (PSS) for Low Acuity Home Care Clients in Thorncliffe Park, Flemingdon Park, St. James Town & surrounding areas

TNO – THE NEIGHBOURHOOD ORGANIZATION (TNO, formerly Thorncliffe Neighbourhood Office) is a community based multicultural, multi-service agency, which offers a broad range of programs to serve Thorncliffe Park, Flemingdon Park, and the surrounding communities through strengthening the quality of individual and community life.

Program Summary

The Personal Support Services (PSS) for Low Acuity Home Care program will assist older adults aged 55+ in Thorncliffe Park and surrounding areas, this includes assisting seniors by mobilizing the necessary Primary Care, Allied Care and Community Care supports. This program will support and enable seniors to age at home, remain connected to health care and support services through integrated service delivery, ensuring seniors are connected to appropriate service providers. This program will also coordinate the provision of travel, interpretation, respite services, primary care, companionship, social/leisure activities and other community health services within our inter-agency and local partner frameworks. Under the direction of the Director of Family and Wellness and the Manager of Family and Wellness and within the mission, vision and values statements and the policies and procedures of TNO, the main focus of this position is to provide highly skilled and professional support to the organization.

Position Summary

Reporting to the Client Service Supervisor, the Homemaker/Cleaner provides assistance with routine activities of daily living, household management, socialization and emotional support for seniors living in their homes. The Homemaker/Cleaner may also provide short-term caregiver relief to those caring for ill or disabled adults. All activities are performed to encourage client independence, support the care plan negotiated with the client and respect client choices.

Main responsibilities:

  • Provides household management and care to clients in their homes by providing assistance to persons who are unable to perform day to day household duties such as like light housekeeping, moderate to deep cleaning, laundry, grocery shopping/ delivery, meal preparation and limited personal care.
  • Encourages and assists clients to perform activities of daily living and community living skills within their capabilities.
  • Assists clients to perform activities of daily living and community living skills within their capabilities.
  • Assesses household environment for safety and security and recommends action to the Client Service Supervisor.
  • Follows care plan as designated and reports any difficulties or changes required.
  • Identifies, observes and reports to issues to the Client Service Supervisor of any circumstances which affect clients and service.
  • Communicates effectively with clients, family members and the care-giving team.
  • Recognizes and respects client’s rights which include informed and quality services and confidentiality.
  • Effectively meets all client services requirements in a courteous and professional manner
  • Proactively address problems, deal with difficult situations, and escalates to Client Service Supervisor as appropriate.
  • Reports any changes in scheduled service for clients to the Scheduling Clerk.
  • Actively participate in Health & Safety initiatives by identifying health and safety concerns, reporting injuries, actively participating in health and safety processes and procedures, maintaining a safe workplace environment and participating in all health and safety training initiatives.
  • Contributes to team effectiveness and service development by documenting care/support at point of service, participate in department meetings, provide input and ideas for improving service and efficiencies, attends all orientation and training sessions, support team decisions and strategies and contribute to department success by taking on special assignments or projects.

Qualifications

  • Secondary School Diploma, GED
  • Home Support programs and related certifications (i.e. Health Care Aid, Food Handler) is considered an asset
  • Experience working in a community setting providing homemaking and household management (e.g., meal preparation, housekeeping, vacuuming, cleaning, laundry etc.)
  • Ability to read, write and speak English; additional language skills are considered an asset
  • Excellent interpersonal, communication, and problem solving skills
  • Ability to exercise good judgment and the ability to work independently, as well as cooperatively with a care-giving team
  • Ability to recognize safety issues and follow safe work practices
  • Access to a reliable car is an asset (mileage will be reimbursed)
  • Must be able to perform physically demanding and repetitive tasks as required including lifting, pushing and pulling using safe body mechanics.
  • Current First Aid certification and CPR certification an asset

 

Working Conditions

 

Hours of Work

Sunday to Saturday from 8am to 8pm, working 35 hours per week, with the flexibility to work all shifts and alternating weekends.

 

Location – This position will be located within any of TNO’s service locations based on community and program requirements. Locations for this position may be added or moved to new addresses in the future according to the community and program requirements.

Vaccination Requirement – As a condition of employment, TNO requires all new hires to provide proof of full vaccination against COVID-19 prior to the start date of employment.
